IT Modification Request

All, per MOC 17-186, we are implementing a new process for requesting IT Modifications.
Attached is a PDF flowchart illustrating how the new process works.

I have also attached two other forms that are now required for all requests.

Both forms are self-explanatory and are essential for us prioritize and improve the return on our IT investments.

For any new requests, please complete the attached forms and email to Bill Savage for review and prioritization by the IT Oversight Committee.

From this point forward, modification requests are not to be submitted directly to Issuetrak.

Please note the process for requesting Support is not changing.
